Making an application

Use this individual checklist to find out if you meet the requirements to apply for a Lottery grant.

Completed application forms must be accompanied by a Verification of Disability form, which is required to verify the extent of the applicant’s disability.

This is provided with the application form and must be completed by a health professional (for example, doctor, practice nurse, occupational therapist)

A letter of support is required from any one of the following

Guidelines for writing the letter of support are attached to the application form. The letter must be written by someone other than the person completing the Verification of Disability form.

Approval process

When your application is received, a letter of acknowledgement will be sent or you may be asked to provide further information.

Decisions are posted to applicants about a week after each Committee meeting. Decisions are not given over the telephone.  If a grant is approved, the applicant will be referred to an assessor with expertise to help them choose a vehicle or equipment that will meet their long-term needs.

Payments of grants are made on receipt of a tax invoice for the vehicle or disability equipment purchased.

When to apply

There are no set closing dates for applications to the Lottery Individuals with Disabilities Committee.  You can submit an application at any time.

You can expect a grant decision within 6 to 16 weeks of an application being lodged and all necessary information being received.

Accountability requirements

The Lottery Grants Board is accountable to Parliament and the New Zealand public for the expenditure of Lottery profits. The Board requires a high level of accountability from all grant recipients.

The Lottery Grants Board monitors the expenditure of all grants. Grant recipients must agree to cooperate with spot checks or on-site investigations of grant expenditure. Individuals are randomly selected for spot checks.

Information disclosure

Applicants should be aware that application information may be released, if required, under the Official Information Act 1982. After 10 years, all applications will be transferred to Archives New Zealand, where they will be available to members of the public.
